Step 1: Enter your USDOT number
We pull your carrier details to pre-fill the form.
Enter your USDOT number and confirm key information such as mileage, address, and EIN (if required). You choose what changes you’re making, and we generate a completed MCS-150 PDF.
Step 2: Your PDF is generated
Review the file name and details.
Once the form is generated, you’ll land on the Success page where your PDF is ready. You can start over at any time to generate a new filing.
Step 3: Pay to unlock download
Secure Stripe checkout.
Complete a secure checkout through Stripe for a small fee (for example, $5). After payment, Stripe returns you to the Success page automatically.
Step 4: Download your PDF
Download access is unlocked after payment.
Once payment is verified, a short-lived download token is issued. That token allows you to securely download your PDF.
