FreightForm Solutions provides a download-only service that generates a completed MCS-150 (Biennial Update) PDF based on information you provide.

We do not submit forms to the FMCSA, AskDOT, or any government agency. You are solely responsible for reviewing, signing, and submitting the form.

2. No Legal or Compliance Advice

FreightForm Solutions does not provide legal, regulatory, or compliance advice. The generated PDF is provided “as-is” based on the data you enter.

You are responsible for ensuring the accuracy and completeness of all information before submission.

3. Payments & Downloads

Payment is required to unlock the PDF download. All payments are processed securely through Stripe.

After successful payment, a short-lived download token is issued to allow access to the PDF.

All sales are final. Because this is a digital download service, refunds are not provided once access is granted.

4. User Responsibilities

By using this service, you agree that:

• You own or are authorized to act on the USDOT number used
• You will review the PDF before submission
• You understand we do not file on your behalf
• You assume responsibility for submission deadlines

5. Limitation of Liability

FreightForm Solutions shall not be liable for any direct, indirect, incidental, or consequential damages arising from the use or inability to use this service.

Our total liability, if any, shall not exceed the amount paid for the service.

6. Availability & Accuracy

While we strive to provide accurate and reliable output, we do not guarantee uninterrupted service or error-free results.

Data pulled from third-party sources (such as FMCSA or SAFER) may be incomplete or outdated.

7. Intellectual Property

The website, software, and generated output are protected by applicable intellectual property laws. You may use the generated PDF solely for your own filing purposes.

8. Modifications

We reserve the right to modify these Terms at any time. Continued use of the service constitutes acceptance of any changes.

9. Governing Law

These Terms shall be governed by and interpreted in accordance with the laws of the United States and the state in which FreightForm Solutions operates.
